1.1 BP may receive, collect, store, use or process personal data from you or any third party.
The types of personal data may include, without limitation:
(i) your personal details, such as name, passport name, age, gender, identity card number, passport number, date of birth, education, race, ethnic origin, nationality, citizenship;
(ii) contact details such as mailing address, email address, contact number(s);
(iii) family information such as marital status, name of spouse or child or immediate family;
(iv) occupation details such as employer’s name, income range, job title, job responsibilities, employer’s contact information and address;
(v) medical and personal health information such as medical and healthcare history, blood type, finger prints, or hereditary characteristics, DNA, health and mental condition, diagnosis, medication and drugs prescribed;
(vi) demographic information such as age group, medical history, genetic characteristics;
(vii) network or Internet protocol address and type of browser you are using, the type of operating system you are using, the name of your Internet service provider and domains used by such providers, mobile network, device identifiers, device settings, browser settings, the BP website(s) you have visited, the type of handheld or mobile device used to view the BP website(s), location information, and the content and advertisements you have accessed, seen, forwarded and/or clicked on;
(viii) credit/debit card number and expiry date;
(ix) billing address;
(x) loyalty program membership details;
(xii) live videos or recordings;
(xiii) CCTV recordings and other images;
(xiv) preferences and interests and other information relevant to patient and/or customer surveys; or
(xv) other information in respect of other BP facilities and/or services used by you.
